It's as easy as 1-2-3...

1. Present or give the Gift Card to the family.

Most often given at or around the Arrangement Meeting or after the selection of an urn/urn vault. It is the card in the frame (one for each service) that states your home is having a young memorial tree planted for the deceased, as part of your personal services and caring for the family.

2. Place the order.

Just fill out the simple order form and fax or mail to Lofty Oaks Association. Fill out all the information on the enclosed order forms (one for each service) so that we can mail the personalized Acknowledgement Certificate directly to the recipient listed.

 

3. We will receive and process the order.

We will send the recipient a personalized Acknowledgement Certificate. The certificate that we send has the deceased's name, the state that the tree is planted in, and your funeral home’s name on it.  This certificate fits right in the same frame that you gave them with the Gift Card.  So be sure to tell them to keep the Gift Card in the frame until the certificate arrives in the mail.
